SQL Server fits organizations with strict data residency, compliance, or hybrid needs. You can run it on Windows or Linux servers, tune the box exactly the way you want, and lean on familiar tools like SSMS and SSRS. It's a common pick where regulators or internal policy require on-premises control.

Azure SQL Database moves that same engine into a managed service. Automatic failover, built-in backups, Hyperscale tiering, and intelligent performance tuning take operational work off your team. Managed Instance offers near-full SQL Server compatibility, easing migration of existing workloads.

On pricing, SQL Server starts at $1,418 per year while Azure SQL Database starts at $0.378 per vCore hour with usage-based, freemium pricing. The two billing models differ enough that direct comparison is tricky.
